Our annual fundraising Banquet is a great time to introduce others to the ministry of Human Life Services and support our important, life-saving work. If you would like to join us on the evening of October 11th and perhaps also like to bring guests who you feel may partner with us prayerfully, as a volunteer, or even financially, please provide their names and contact information below (please provide your name and contact information as Guest 1, and so forth).
Should your guests have any special needs (ex. wheelchair, special diet, etc.), please indicate that as well.
Please note that this form is only a request to attend our Banquet and does not automatically guarantee a seat until you receive confirmation from an HLS Team member.
